PATIENT BILL OF RIGHTS AND RESPONSIBILITIES


SurgiCare of Central Jersey. Inc. believes that respecting the rights of patients provides each patient with a sense of security which will enhance the effectiveness of patient care. These rights are:

  1. The right to treatment without discrimination as to race, religion, national origin, sex, age, disability, or source of payment.
  2. The right to considerate and respectful care.
  3. The right to obtain from the physician complete, current information concerning diagnosis, treatment, and prognosis in terms that the patient can be reasonably expected to understand in order to provide informed consent for procedures. When it is not medically advisable to give such information to the patient, the information should be made available to an appropriate person' in the patient's behalf.
  4. The right to participate in health care decisions.
  5. The right to know the name and title of the physicians and other health care providers responsible for their care.
  6. The right to refuse treatment.
  7. The right to expect that all communications and records pertaining to his/her care will be treated confidentially. Except when required by law, patients have the right to approve or refuse the release of records.
  8. The right to be advised of any proposed experimental treatment and the right to refuse such treatment.
  9. The right to examine and receive explanation of bills and charges.
  10. The right to receive information concerning advance care directives.
  11. The right to appropriate assessment and management of pain.
  12. The right to personal and informational privacy, within the law.
  13. The right to discuss or submit in writing concerns and grievances regarding care received at SurgiCare. Such concerns may also be addressed to:

In order to assist the SurgiCare staff in providing safe, effective and efficient patient care, the patient shall accept these responsibilities:

  1. Responsible for providing, to the best of your knowledge, accurate and complete information about present health status and past medical history, and reporting any unexpected changes to the appropriate practitioner.
  2. Responsible for following the treatment plan recommended by the physician involved in the procedure.
  3. Responsible for providing an adult for transportation home after surgery and an adult to be responsible for you at home for the first 24 hours after your procedure.
  4. Responsible for indicating whether you clearly understand a contemplated course of action and what is expected of you.
  5. Responsible for your actions if you refuse treatment, leave the facility against the advise of the physician, and/or do not follow the physician's instructions relating to your care.
  6. Responsible for assuring that the financial obligations of your health care are fulfilled as expeditiously as possible.
  7. Responsible for providing information about and/or copies of any living will, power of attorney, and other advance directives that you desire us to know about.
